While most European leaders have responded to the U.S.-Israeli strikes on Iran with condemnations of the Iranian regime and tepid calls for “de-escalation” designed not to offend Washington, Spanish Prime Minister Pedro Sanchez has unequivocally condemned the war on Iran as a breach of international law.
It has been said many times…everything Trump touches dies. There is a long history that confirms this. That said, he has somehow been politically successful which I think says more about the electorate than him.
I think the possibility of this causing Spain to leave NATO is less than 0%.
If Spain left NATO, then even the US could attack [Spain] and no one would be obligated to come to their defense.

NATO was created to ward off the USSR which is no longer with us. Now the big bad bear is the Russian Federation which is being dismantled by Ukraine. Maybe NATO is obsolete.
