… what do you expect? That 15% skim rate to the insurer has to come from somewhere. And poorly educated rural residents are more likely to fall for the Medicare Advantage scam.

When I first retired, I considered moving outstate, to get away from the traffic and crowds in metro Detroit.

Checked the ACA plan rates, and got a surprise. Premiums were significantly higher in smaller cities, like Kalamazoo and Allegan, vs Grand Rapids or metro Detroit.

When I lived in Texas, premiums were at least 1/3 cheaper in West Texas vs. Houston and Dallas. Of course, in a lot of those small West Texas towns, you were so far from a hospital that the patient never made it there in time to run up much of a bill.
